Banner/logo - FC All Starz (107 UP'S!)

Can anyone create a new logo and banner for me?

I have a few requests for my logo.
1) It has to include one star above the crest.
2) It has to include my team name (FC All Starz).
3) It has to include the year when I was founded (2013).
4) The only colours it can contain are bright blue and white.

I also have a few requests for my banner.
1) It must contain my logo.
