Substitutions not taking place

I have set for substitutions to take place in the 89th minute (to keep player morale up) but for the last few games these substitutions have not taken place. Has anyone else had the same issue?

Cancel that, I realise my error. I changed the substitutes but not the adjustments, so had players coming on who weren't even on the bench!
